Dr. Tillson was born in Darke County. He graduated in 1862 from the Cincinnati Eclectic Medical Institute. He practiced in West Alexandria, Ohio. The ledger lists visits and accounts of patients.

The collection contains a single ledger used by Dr. Tillson to record information transferred from other ledgers concerning his patients, visits, and accounts therein. The ledger was also used by his wife after his death to record grocery lists of different people.
